As a parent/guardian of Upward Bound Christian Preschool I/we will be driving for some preschool field trips during the 2009/2010 school year. When driving on a preschool field trip I understand that all children traveling in my vehicle will be seat belted at all times. Children under the age of four will be placed in and seat belted into a booster seat. I also agree that I am an adult of legal driving age with a valid driver's license including automobile insurance. I also agree that when driving for a preschool field trip I will be free of alcohol or any of substance that would interfere with my judgment and driving.
