I give ________________________________________ permission to travel on field trips with Upward Bound Christian Preschool. Dates and destinations will be posted on the calendar prior to field trip days. Students will be in a seat belt at ALL times while traveling in a vehicle. All drivers will have a valid license and insurance. On field trip days, the students will be dropped off and picked up at regular time and location unless otherwise stated. The teachers will post cell phone numbers on door if contact is needed during the field trip.
